Preparing for Consultation
When you go in for your consultation with Dr. Sadiqa Stelzner, MD, you are going to be asked to complete a new patient registration form as well as a medical history form. If you do not have the completed form, then you will have to complete it when you arrive.
It will be during your consultation that you will have a complete comprehensive eye exam which will help to determine your overall suitability to have refractive eye surgery or Lasik. It will be this eye exam that will include a microscopic exam of your eye, as well as complete and accurate measurements the thickness and shape of your cornea, and then the surgeon will apply trial lenses which will stimulate the outcomes of refractive surgery.
If you end up being a good candidate for surgery, then your surgeon will determine the type of level of correction for your vision that you will need. You will also be advised on the type of outcome that you have, and you will be made completely aware of any implications for correction your vision with laser surgery. You will need to plan on staying at the laser eye surgery clinic for around 2 hours. You are not going to be charged for this visit.
Whenever you come to your consultation, you will need to bring the following:
Medical history and new patient registration forms: These will help to speed your visit along and you are able to download them and then have them printed on the form links listed below and then simply fill them out before you arrive.
Eyeglasses: You should bring your eyeglasses that you wear the most and/or the prescription contact lenses that you have, even if they don’t help your vision any more. Old prescription contact lenses and eyeglasses are going to be really helpful as they will be able to provide a lot of really important information on the past condition of your eyes.

If you are coming in for refractive vision correction, you will not be charged for the visit, but this type of correction is SMILE, LASIK or PRK. If you happen to be interested in cataract surgery or another type of procedure, then you will be charged for the initial consultation.
