On the day of your consultation, you will be asked for the completed new patient registration and a medical history form. If you do not have the completed form, you will need to complete it upon arrival.
During your consultation, you will receive a comprehensive eye examination to determine your suitability for surgery. The comprehensive examination includes a microscopic examination of the eye, advanced diagnostic imaging to assist Dr. Hamilton in determining the overall health of the eye structures, and a detailed discussion of the treatment options and Dr. Hamilton’s recommendation as to which procedure best fits your needs.
If you are a candidate for surgery, the doctor will determine the specific level of vision correction you require. You will also be advised about your likely outcome and made fully aware of the implications of the procedure. Plan on staying in the clinic for at least one to two hours. If you are coming in for a refractive vision correction consultation (this includes LASIK, PRK, SMILE, or EVO ICL), and have not had any previous vision correction surgery, you will NOT be charged for this visit. However, if you have had previous vision correction surgery or have known eye disease, there will be a charge for the consultation or, as appropriate, the consultation will be billed to your medical insurance. Hamilton Eye Institute does not take vision insurance (e.g. VSP).
Please bring the following to your initial visit:
- Eyeglasses: Bring the eyeglasses you wear most often and/or your contact lens prescription with you, even if they no longer help your vision. Old eyeglasses and contact lens prescriptions are also helpful to bring as they will provide important information about the past condition of your eyes.
- New Patient Registration and Medical History Forms: To help facilitate your visit, you can download and print the New Patient Questionnaire and Medical History forms below and fill them out before your arrival.

Special Preparation for Contact Lens Users
Remove your soft contact lenses at least 3 days before your visit and keep them out until the appointment is over. Rigid gas permeable (RGP) and other hard contact lenses should be kept out for 3 weeks prior to the consultation.
